* Eligibility for selected procedures will be assessed by a doctor based on a questionnaire completed by the client (without a personal visit to the doctor). Only in the event of a need to adjust the treatment plan for health reasons will the client be invited to a free medical examination. The completed questionnaire must be submitted no later than upon arrival.
